Dr Khattar is a Urologist who has dedicated himself to the fields of Reconstructive Urology, Female Urology and Neurovesical dysfunction. He is a member of Female and Functional Urology Subspeciality section of the USI. He has been a core member for Indian guidelines committee of the Urological Society of India (USI) for Adult Urinary Incontinence and Urethral Strictures.
Dr Khattar has worked as Professor of Urology at the PGIMER and Dr RML Hospital, New Delhi where he was actively involved in teaching and research. No, Stricture is caused by scarring of the tissues and it cannot be treated by medication. It always requires mechanically opening the blocked passage again either endoscopically or surgically. the medicines can only reduce associated infection or inflammation around the scar and can provide a little relief but they wont open the stricture
we have come very far from previous notions of "once a stricture, always a stricture". Smaller strictures (<1.5 cm) are mostly curable with endoscopy, largers or recurrent ones, however, require reconstructive surgey which depending on the type of stricture, can give cure rates from 60-90% or even more.
fistula after a hysterectomy is an abnormal opening between bladder and vagina causing continous leak of urine. it is called vesicovaginal fistula or VVF. In most cases it can be corrected with a Vaginal surgery which does not require going through the abdomen as in laparoscopic or robotic and saves cost too!!
Neurogenic bladder can only be managed. It cannot be cured. However, there are chances for spontaneous recovery which depends on the type and nature of the basic neurological condition.
Botox Injections in the bladder are given to keep the bladder relaxed and allow safe storage of urine without leak and without damaging kidneys because of high pressure if medicines alone are not enough. The effect of botox injections lasts for 4-6 months and they need to be repeated.
Not doing or improperly doing self cathetrisation can cause more severe UTI than self cathetrisation. CIC has been proven to be safest and most effective long term method to take care of bladder emptying in cases of neurogenic bladder.
